An Integrated Health Care Approach


The Hub Health Collective is comprised of a diverse group of inter-disciplinary team members, inclusive of:  Psychiatrists, General Practitioners, Nurse Practitioner, Pharmacists and Licensed Practical Nurses, Register Nurses, Counsellors and integrated systems planners.  


Why has the Hub Collective come together?

Increasing demand for quality, integrated care coupled with limited resources and coordination across systems and sectors, continually generate barriers for both patients, who require streamlined care to address complex mental health, substance use, primary care, and other social care support needs and for health care professionals to ensure streamlined care is provided to meet the integrated needs required for person-centred care. 

Acknowledging this persistent challenge, a group of caring health and social care professionals came together through building collective agreement that the current health-care service delivery methods require a different approach, that can be offered through strong, inclusive leadership to integrate care across systems to:

  • Improve health and wellness outcomes for patients through person-centred care,

  • Increase effectiveness of health professionals to provide streamlined care,

  • Reduce barriers by offering adaptable and responsive environments of care (e.g., hospital, community, outreach, and virtual),

  • Enhance collaboration across the health, housing, and community support systems to integrate services and referral pathways in an efficient and timely manner, and

  • Deliver cost-savings and coordination of effective health service delivery.


Generating Solutions to Complex Needs Through Care and Empathy

Hub Health Collective introduces efficiencies to traditional systems of care by fostering an integrated care approach that is grounded in high standards of care, technology advancement and social innovation that fosters an environment for a patient to strengthen their resiliency and agency within their own care plan. 

Integrated health care is achieved when a whole systems approach that is grounded in the local resources and strengths of a community is activated to respond to individual’s needs in a comprehensive and effective manner. 

Collaborative work among all the health and social service providers has shown to have immediate patient impact, in regards to retention and therapy completion. Collectively meeting the patient where they are on their path to recovery, has proven to have positive measurable outcomes. 


Hub Collective Contributes to Community Resiliency and Health Outcomes in Multiple Ways

Coordinated and integrated care planning,

  • Ensuring that referral and support pathways across systems are actively responding to the patient needs in a timely manner,

  • Consistent communication and follow up between care providers to ensure health and wellness outcomes are being achieved.

  • Seeking sustainability, transformation and active partnering in the community to increase local capacity to deliver integrated health and social care service to individuals with complex needs.
